Description

GSK Access is a program sponsored by GlaxoSmithKline that provides GlaxoSmithKline prescription medications at no cost to Medicare Part D Prescription Drug Plan enrollees who meet the eligibility requirements. Eligibility is based on annual household income and proof that the applicant has spent $600 or more for prescription medicines for the year. The patient must have Medicare Part D, and have an income at or below 250% of the Federal Poverty Level. The patient must also have spent $600 dollars on medications through the Medicare Part D plan. Anyone requesting assistance can call to request a faxed application or download it from the website. The application will be faxed out. The completed application must be mailed back. The patient is notified of eligibility for the program.
