This page contained detailed instructions on how someone could submit a block to honor a loved one living with Huntington's Disease for the Phase III Quilt. The Phase III Quilt was unveiled in 2006 and, currently, there are no plans for a Phase IV Quilt.
 
If you submitted a block to Deb Hart for the Phase II HD Quilt Project, the biography may be been lost.  Please check the list of names where a bio is needed to be included in the Phase III Quilt.
 
Application form, including all instructions, to be filled out by any person or organization wishing to borrow the Quilts for an HD event/fund raising project.
Quilt Schedules
All of the HD Memorial Quilts are available for loan to support HD events. The  current schedule for the loan of the Quilts is maintained by Sue Hodgson's.  Please contact Sue if you want to check out the availability of any Quilt for your event date prior to submitting a Loan Application.   Email: Sue Hodgson
