Loan of the HDSF Huntington’s Disease Memorial Quilts
For Huntington's Disease Awareness and/or Fund Raising Events

Currently, all three Phases of the Huntington’s Disease Memorial Quilts are available
for loan to any HDSA Chapter, Support Group, Center of Excellence or HD family supporting an HD awareness, educational
or fund raiser event.
Loan of the Huntington's Disease Memorial Quilts (hereafter called the Quilts) will be made on
a first-come-first-choice basis. You may request all three Quilts for your event. However, if there are requests
for a particular Quilt within the same timeframe, one Quilt will be sent to each location.
Those borrowing the quilt will be responsible for insured shipping charges associated with transporting
the quilt. The above form includes the actual Loan Application as well as:

Cost to ship a Quilt UPS for your event [you can pre-pay by Pay Pal, see below]

Information on the size and weight of the Phase I, II and III Quilts

Instructions for the borrower on displaying, handling, and collecting donations.

All Quilts are suitable for hanging and will arrive in its own protective duffel
bag along with the Quilt book of biographies, a Guest Book for comments from attendees at your event, and wooden hangers.
All of these items must be returned with the Quilt.
To reserve one the Quilts for a Huntington's Disease event please fill out the Loan Application, attached,
and mail it per the instructions on the application. Please include this application, and as much detail as possible in order
to provide the Quilt Project with the most complete information. E-Mail or mail the application to:
